Please use the link to your Patient Portal below and follow the brief instructions at the bottom of this page as a guide to access your information. Written requests for Medical Requests can be faxed to 209-348-9452. 

Family Care Center

- Click on the BLUE 'PATIENT PORTAL LOGIN' BUTTON 
- Click on the 'REPORTS' Tab 
- Select 'ALL' (this will bring up everything in alphabetical order in your chart including all of Dr. Kenny's chart notes, all previous consult notes from other providers, Lab Reports, and all radiology reports that are currently in your chart) 
- Click on 'Other Reports'
- Select the Green Magnifying Glass
- Select the Green Down arrow to Download the document
